Manifest för integrativ medicin antogs i Florens

Integrativ medicin grundar sig på en icke-hierarkisk sjukvårdsorgansiation, där varje patient kan delta i sin egen vård.  Det är ett par av de centrala principerna i ett manifest om integrativmedicin, som antogs i Florens, Italien i december 2011.

Manifestet lanserades aktivt under den nyss avslutade Femte internationella kongressen om integrativmedicin i Florens.

Kan få begränsat genomslag
Initiativet att formulera manifestet kom från det Italienska sällskapet för homeopati och integrativ medicin (SIOMI) och stöddes enligt uppgift av en rad såväl offentliga som privata aktörer och organisationer.

I samtal med I C tonade andra initierade talespersoner ned betydelsen av Manifestet mot bakgrunden av Italiens mångfacetterade hälso- och sjukvårdsorganisation. Man framhåller regionernas höga grad av självbestämmande att sköta vården tillsammans med staten. Manifestet bör hittills ses enbart som ett regionalt initiativ.

Trots detta är dokumentet tveklöst ett intressant dokument, som omfattar många aspekter av integrativ medicinen, inklusive globala principer, etik, organisation, juridik och filosofi.

Främjar harmonisering och kontinuitet
Integrativ medicin har till syfte att uppnå vårdkontinuitet och smidig harmonisering av alla hälso- och sjukvårdsresurser, oavsett medicinska specialiteter, den historiska tidpunkten eller samhället där det verkar. De huvudsakliga hälso- och sjukvårdsresurserna definieras som biomedicin respektive komplementär- och alternativmedicin (KAM).

Antar gemensamt ansvar
Integrativ medicin ser till individens komplexitet och helhet. Den omfattar vetenskapen bakom dessa viktiga frågor och antar ett gemensamt ansvar för individens omvårdnad och hälsa.  Sjukdom som fenomen betraktas som resultatet av många olika faktorer; strävan efter hälsa prioriteras.

Splittring ett hinder att övervinna
Integrativ medicin erkänner att individen är en kombination av kropp, sinne och själ med hänsyn till hennes egen personliga historia och den omgivande miljö. Den betraktar den nuvarande splittringen mellan olika terapeutiska modeller som ett hinder, som måste övervinnas för att uppnå gemensam vård och ett fruktbart utbyte.

Skyddar valfriheten
Medborgarens rättighet att välja hälso- och sjukvård bör upprätthållas och försvaras med hänsyn också till läkarens frihet att ge råd. Varje individs personliga, kulturella och andliga övertygelser påverkar medborgarens erfarenheter och tolkning av både sjukdom och helande.

Tar hänsyn till mening
Integrativ medicin tar hänsyn till de meningar som vi ger hälsa och helande respektive sjukdom och behandling mening. Varje patient är både det subjekt som påverkas av sjukdomen och personen som kan uttrycka sin potential för att hela sig själv med lämplig handledning och ändamålsenligt stöd.

Främjar forskning
Manifestet om Integrativ Medicin främjar forskning om hälso- och sjukvårdens effektivitet och säkerhet, oavsett de många, varierande angreppssätten inom klinisk praxis. Disciplinerna inom modellen för integrativ medicin måste säkerställa en en teoretisk underbyggnad på korrekt nivå och praktisk säkerhet och effektivitet.

Integrerar värden
Integrativ medicin för samman – integrerar  – värden som uttrycks av medborgarna med de yrkesmässiga värderingarna hos läkare och annan hälso- och sjukvårdspersonal i ljuset av social rättvisa och hållbarhet i samhället.

Skapar synkretiskt forum
Integrativ medicin skapar ett synkretiskt forum inom de olika medicinska disciplinerna. Behovet finns på grund av de nya kunskaperna om den levande organismen som uppnåtts inom de senaste decennierna. Paradigmet integrativ medicin grundar sig på ett tvärdisciplinärt angreppssätt för hälso- och sjukvårdsorganisation snarare än ett hierarktiskt angreppssätt.

Författarna till manifestet är Simonetta Bernardini, president i SIOMI, Italiens sällskap för homeopati och integrativ medicin, Ivan Cavicchi, Avdelningen för medicinsk filosofi och psykologi, Tor Vergatauniversitetet i Rom, Alfredo Zuppiroli, Chefen för den regionala biotetiska kommittén, Toscanaregionen, Andrea Dei, Institutionen för kemi, Florensuniversitetet, Francesco Macrì, professor, Sapienzauniversitetet i Rom och Guido Giarelli, biträdande professor i sociologi, medicinska fakulteten associate professor in sociology, Magna Græcia-universitetet i Catanzaro. Alla dessa undertecknande personer är också legitimerade läkare.

Personalized Medicine Here Say Speakers at ECIM 2012

Personalized medicine has become a catchword for matching treatment to genetic and other biomarkers. But there is much more to it than that. It is also about providing real treatment choices, like they do in Tuscany, Italy.

In Tuscany, people can choose their own combination of conventional and complementary medicine. Hence, the Congress venue is no accident, said Dr M Silvestri from the Regional Health Department, when the 5th European Congress on Integrative Medicine (ECIM) opened on Friday September 21st in Florence, Italy.

From ongoing tensions…

Of course, non-conventional medicine has not made equal headway elsewhere, and other speakers made reference to the ongoing tension between paradigms and processes across countries and cultures, the chief victims of which are of course, the so called end-users or patients.

The three-day Congress, attracting some 700 participants—e.g. researchers, physicians, physiotherapists, pharmacists— from 30 countries presenting some 500 scientific papers, is a historic first for ECIM, which has never before been held outside Germany.

I C is represented with presentations, posters and other forms of scientific input.

The theme of the Congress is ”The future of comprehensive patient care. Promoting health and developing integrated and sustainable treatment for acute and chronic diseases.”

…towards personalized medicine

Moving beyond the tensions between CAM and conventional medicine, the Dean of Florence University Faculty of Medicine, presented a vision for tomorrow’s health care system. In line with the 7th European Framework Programme, medicine needs to focus on four P’s: Predictive, Preventive, Personalised and Participatory, says Professor Gensini.

The 4P Medicine concept originates from the area of marketing. Professor Gensini explained that the application of the 4Ps is really about a renewed attention to narrative medicine and personalised treatments, elements that are also key aspects of integrative care.

Professor Gensini further stressed the importance that our health care systems get used to constant changes. We need to move from a focus on disease to a focus on individuals, from cultures of monoprofessionalism to cultures of multiprofessionalism and, from taxonomic approaches to problem- and process- based approaches.

…or person-centered medicine

However, at the ECIM Congress in Florence, Paolo Roberti di Sarsina taly Observatory and Methods for Health, Department of Sociology and Social Research, University of Milano-Bicocca, Bologna , Mauro Alivia and Paola Guadagni of  the Charity Association for Person Centred Medicine, Bologna introduced the concept of  person-centred medicine as opposed to personalised medicine, a term currently used to exclusively denote an approach based on pharmacogenomics, proteomics, metabolomics and physiomics.

Pharmacogenomics is extremely important and often overlooked. The field was pioneered globally by among others Sweden´s grand old man of clinical pharmacology Folke Sjöqvist, Huddinge Hospital. Personalized medicine has been developed from the realisation that patients diagnosed with the same disease often respond differently to drugs, and so treatment can be tailored to an individual´s genetic profile.

However, human nature is more complex than our genetic make-up, the Italian authors say. It includes psychological and spiritual aspects as well as physical ones. To be individualised, diagnosis and treatment need to take into consid- eration the human being in their full expression. The challenge for the future of medicine therefore extends beyond biomedicine. The whole health system needs to set in motion a process whose central aim is the global care of the individual patient. This is person-centred medicine, they assert, thus preferring to use another term rather than trying to give the term personalized medicine a wider meaning.

Topics at the Congress

Central topics on the Congress programme are the effectiveness of complementary medicines and practices, their integration with orthodox medicine and the future development of integrative medicine.

Sub-topics of clinical relevance include integrative oncology, acute and chronic pain, mental disorders, atopic and allergic diseases, gender medicine, and paediatrics. Other subjects of interest are the integration of complementary medicine in the healthcare systems in Europe and the European regulation for herbal and homeopathic drugs.

For the first time, the exciting transdisplinary field of psychoneuroimmunology has been included as well as integrative veterinary medicine and wellness techniques/ practices.

Ideal of humanization
One of the aims of ECIM 2012 is the creation of a network of researchers and physicians. A major objective is to develop a basis for future clinical and research guidelines, which is – as the organizers write in the program – an ambitious project considering ongoing differences within complementary medicine and communication difficulties with mainstream medicine.

While these differences lessen day by day, they still exist. ”Nevertheless, we firmly believe that this aim should be constantly pursued to achieve the therapeutic ideal of medical humanization in everyday clinical practice,” the organizers conclude.
